Looking for a new way to take on relapse prevention?
Introducing our Relapse Prevention Plan!
Newly updated, this is a 4-page worksheet that helps people in recovery maneuver through difficult moments: cravings, stress, and vulnerability.
Page one focuses on identifying warning symptoms and triggers, reviewing negative effects of drugs/alcohol, and the worthwhile aspects of staying clean/sober.
Page two is aimed at preparing for stress and cravings ahead of time. It’s serious, it’s funny, and it’s worth reading.
Page three takes on emergency urge management, i.e. ways to avoid using.
The fourth page provides link information for booklets to help with other aspects of addiction and recovery.
Ideally the handout would be filled out in advance, but it serves as an acute rescue packet too. Its checklist format makes it easy to use during life’s rougher moments.
There is no fine print. It’s really free, as we think it can benefit many people. You won’t be charged! Download, save it on your computer, and print up as many copies as you’d like. Make sure to save it as soon as you get the link, though, as the link sometimes expires.
This worksheet is written for for the general public as well as for substance abuse counselors to use with clients.
